What is an ACO?

An Accountable Care Organization (ACO) is a group of health care providers that affiliate to share responsibility for the medical care and management of patients. By collaborating in a management structure designed to promote efficiency and reduce waste, the ACO controls costs and increases the quality outcomes.

Choice Accountable Care Organization & Education System, Inc. (CACO-ES)

A CMS Medicare Shared Savings Program (MSSP) Participant Organization since January 2013. As of January 1, 2024, a participant organization under Live Oak Care ACO. Currently, the CACO-ES has approximately 10,000 traditional Medicare and dual eligible (or those that qualify for both Medicare and Medicaid) beneficiaries. We have increased the CACO-ES’ attribution over 80% since January 2013, when we started with the Medicare Shared Savings Program. Today, we care for Medicare and dual-eligible beneficiaries throughout the Atlanta metro area, operating from 160 health center locations and two mobile medical offices.

Our goal is to improve life and health through sustainable collaboration of community partners. We practice “sought-after” standard for health delivery integration and have found this renders improved patient experiences.

We Accomplish Our Goals Through Collaboration:

  1. Transform the organization with a structured plan of care process.
  2. Improve the delivery of health care of beneficiaries assigned to the CACO-ES, eliminating waste and providing more provider support.
  3. Control the financial costs projection by lowering the costs of health care without decreasing the quality in health service and outcomes.
